The Caregiver Support Program (CSP) is one of the Veterans Health Administration's most rapidly growing service areas, as a result of infrastructure modernization, programming innovation, and expansion of population served by the Program of Comprehensive Assistance for Family Caregivers (PCAFC) under the MISSION Act. The Caregiver Support Program (CSP) Social Worker assists in the day-to-day operations of the Caregiver Support Program and ensures compliance and collaboration with local facility and VA Central Office Caregiver Support Program initiatives. The incumbent collaborates with the other members of the CSP team in the provision of comprehensive health care services to Veterans, ensuring equity of access, service, and benefits to this population, and ensures the care provided is of the highest quality.
The incumbent develops and facilities caregiver support groups and educational programs and distributes educational materials provided by VA, local, state, and national caregiver organizations. The incumbent participates in direct caregiver and Veteran contacts to assess caregiver stress/burden and the development application of directed clinical treatment interventions. The incumbent is responsible for conducting regular home visits to Veterans and Caregivers participating in the PCAFC, responding to calls from the National Caregiver Support Hotline, responding to other phone calls and general inquiries regarding the CSP, and assisting with general tasks associated with the program.
The incumbent collaborates with interdisciplinary teams throughout the medical center on caregiver issues. They are responsible for creating educational tools, developing programs, and implementing training focused on specific caregiver needs/issues and providing education and in-service training to VAMC employees, Veterans families, and caregivers. They also provide ongoing consultation and support to colleagues regarding caregiver support services and issues facing Veterans, families and their caregivers.
The incumbent demonstrates skill in communication and can develop positive, effective relationships with all CSP stakeholders including Veterans, caregivers, local, state and federal agencies, as well as non-governmental organizations. They provide referrals to VA medical and mental health treatment and to other VA services including VA benefits, as well as community-based social services and other non-VA entitlement programs.
The incumbent assists in coordinating events and programs for the annual National Family Caregivers Month in November, which includes announcements and advertising of month-long activities, mobilization of participants, educational sessions, caregiver recognition events, and caregiver support programs. In collaboration with other clinical staff and program managers, the incumbent will establish a caregiver support protocol (resources, education and training) sensitive to the issues of the caregivers who are providing care to our youngest returning Veterans.

+ Ability to assess the psychosocial functioning and needs of Veterans and their family members, and to formulate and implement a treatment plan, identifying the Veterans problems, strengths, weaknesses, coping skills and assistance needed.
+ Ability to implement treatment modalities in working with individuals, families, and groups to achieve treatment goals .This requires judgement and skill in utilizing supportive, problem solving, or crisis intervention techniques.
+ Ability to establish and maintain effective working relationships and communicate with clients, staff, and representative of community agencies.
+ Fundamental knowledge of medical and mental health diagnoses, disabilities, and treatment procedures. This includes acute, chronic, and traumatic illnesses/injuries; common medications and their effects/side effects; and medical terminology.
+ Knowledge of community resources, how to make appropriate referrals to community and other governmental agencies for services, and ability to coordinate services.
+ Skill in independently conducting psychosocial assessments and treatment interventions to a wide variety of individuals from various socio-economic, cultural, ethnic, educational and other diversified backgrounds.
+ Knowledge of medical and mental health diagnoses, disabilities and treatment procedures (i.e. acute, chronic and traumatic illnesses/injuries, common medications and their effects/side effects, and medical terminology) to formulate a treatment plan.
+ Skill in independently implementing different treatment modalities in working with individuals, families, and groups who are experiencing a variety of psychiatric, medical, and social problems to achieve treatment goals.
+ Ability to provide consultation services to new social workers, social work graduate students, and other staff about the psychosocial needs of patients and the impact of psychosocial problems on health care and compliance with treatment.
The full performance level of this vacancy is GS-11. The actual grade at which an applicant may be selected for this vacancy is in the range of GS-9 to GS-11.
