We are seeking a motivated and experienced
Healthcare Marketing Liaison to join our team at a fast-paced, high-acuity skilled nursing and rehabilitation facility located in Island Park, NY (this is a field-facing role though). This role is ideal for a candidate who is passionate about building relationships, increasing Medicare referral volume, and driving strategic census growth.- Cultivate and manage referral relationships with hospital discharge planners, case managers, physicians, and managed care partners, with a strong emphasis on Medicare-eligible referrals
- Identify, qualify, and convert referral opportunities to meet and exceed short-term rehab and Medicare census goals.
- Serve as the first point of contact for inquiries and referrals, including evenings and weekends as needed
- Collaborate with clinical and administrative teams to review cases and determine appropriate placements
- Maintain consistent visibility in Long Island hospitals, especially within Nassau, Queens, and Suffolk, while also expanding outreach to broader healthcare networks as needed
- Provide timely and professional communication with families and referral sources throughout the admissions process
- Minimum 3-5 years of experience in healthcare marketing, admissions, or liaison work (preferably in a skilled nursing, short-term rehab, or subacute care setting in the Long Island area
- Demonstrated ability to increase Medicare referrals and develop high-quality referral pipelines
- Strong working knowledge of admissions processes, Medicare/Medicaid, and managed care insurances
- Existing relationships in Long Island hospitals are highly preferred
- Ability to confidently communicate clinical offerings and patient outcomes
- Must have a valid driver�s license and reliable transportation for regular in-person outreach
- 7. Comfortable using EMR systems, and Referral management portals like Careport
