Position Summary


Provides specific tailored services to Native Hawaiian individuals and families on the island of Oahu to improve a client/patient’s quality of life. Under the direction of the Client Services Manager, the Mobile Coordinator (MC) carries out functions within culturally, appropriate service delivery in the community that include client identification; coordinates the utilization of the KOM mobile unit within communities at events, activities and other outreach/promotional opportunities. The MC serves as a “bridge†between health care consumers and providers to improve health and promote healthy lifestyles. This position works together with the Community Health Workers within KOM to meet community and grant needs.


Essential Duties and Responsibilities


1. Identifies the needs of the community to allow Ke Ola Mamo services as outlined in grant goals and objectives.


2. Plans and coordinates the use of the mobile unit in community activities.


3. Travels throughout the island of Oʻahu in partnership with KOM staff and community partners to promote the mobile units utilization.


4. Collaborate with agencies to offer supportive services, research, and resources available to them with the mobile unit.


5. Increase visibility of KOM throughout Oʻahu and especially in high Native Hawaiian communities.


6. Keeps careful records of all contacts made and follow-up as necessary, providing referral support as needed.


7. Completes intakes as needed in the community, while community members wait for screenings.


8. Speaks before neighborhood groups to establish communication and rapport between people in the community and Ke Ola Mamo, to publicize supportive services.


9. Attends community meetings, health fairs, and represents KOM as requested by Client Services Manager; to understand community issues and build relationships with community members.


10. Networks with community agencies and attends community meetings.


11. Distributes flyers, brochures, and/or other informational or educational documents to inform members of the Native Hawaiian population.


12. Ensures confidentiality of client records.


13. Maintains accurate tracking of the upkeep, services, and activities engaged in use of the mobile unit.


14. Works with all levels of Administrative and Clinical staff to develop and implement new ways to access health.


15. Ensures adherence to federal, state, and local laws, rules, and regulations.


Other Duties and Responsibilities


1. Attending monthly staff meetings.


2. Routinely uses KOM database and other computer systems including e-mail, internet, word processing, and electronic health records (EHR).


3. Performs other related duties and responsibilities as assigned by the Client Services Manager.


4. May collect data to help identify community health needs.


5. Attend ongoing training.


6. Provides monthly status report to Client Services Manager.


Working Conditions


Indoors and outdoors, under a variety of conditions in the community.


Working Hours


Normally 8:00 a.m. to 5:00 p.m., Monday through Friday. Some evening and weekend work may be required, depending on the requirement of position duties and responsibilities.

Computer, copier, telephone, and other standard office equipment; KOM 7 passenger van; KOM mobile unit

The employee must occasionally lift and/or move up to 25 pounds; assist clients with mobility problems. Specific vision abilities required by the job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus.

1. Demonstrated knowledge of Native Hawaiian cultural values and practices in the delivery of health care services.


2. Knowledge of community, health and human service agency resources.


3. Knowledge of community needs.


4. Knowledge of technology and automation.


5. Knowledge of the principles and practices of effective community health outreach and case management.


6. Working knowledge of software applications used by KOM: ECW.


7. Skill in integrating Native Hawaiian cultural sensitivity methods in delivery of service.


8. Ability to advocate on behalf of the client by means of locating resources to meet as many of the client’s needs as possible and connecting the client to identified resources; to assess the client’s progress and the effectiveness of the treatment plan interventions; to ensure the client receives services in a timely and effective manner.


9. Ability to document interactions with clients and service providers, to facilitate communication.


10. Ability to address the public and professional groups.


11. Ability to perform outreach services.


12. Ability to prepare and maintain reports and records.


13. Ability to work effectively and independently with various levels of Clinical and Administrative managers, coworkers and staff within KOM and with community leaders and provider/agency representatives.


14. Ability to establish and maintain effective working relationships with employees, other agencies and the public.


15. Ability to maintain composure under time restraints and when serving clients.


16. Ability to maintain accountability and confidentiality.


17. Ability to prioritize tasks, good time management skills and demonstrate high integrity.


Other Requirements


1. Have a valid driver’s license and use of a personal vehicle for work with proof of adequate automobile insurance coverage. Will use company vehicle for business of the organization.


2. Driver’s abstract without any moving violations within the past three (3) years.


3. Valid tuberculosis clearance within one (1) year.


Education and Experience Requirements


1. Associate or bachelor’s degree in human services preferred, but not required.


2. Minimum 2 years of general experience. Work experience which involved meeting and dealing effectively with people and which demonstrated the ability to (a) establish and sustain positive relationships with people; (b) provide and elicit pertinent information; and (c) recognize and identify basic human needs and motivations.


3. Minimum 2 years of experience working with Native Hawaiian population, community agencies, providers and insurers.


4. Computer literate (working knowledge of Microsoft Office applications) and ability to learn KOM’s Electronic Health Record.


5. Any combination of education and experience would provide the necessary knowledge, skills and abilities to perform the essential functions of this position.
