The HIM/Medical Records Clerk following hospital policies and procedures, and federal and state regulations, completes release of medical record information including retrieving patient’s medical charts and returning charts; copying/scanning and forwarding medical records accurately and correctly according to established medical record policy and HIPAA guidelines.


Job Description:


  • Retrieves medical records upon request from authorized staff members, medical staff, and/or administration, and as appropriate for the working needs of the facility and patients in a timely and courteous manner.


  • Receives incoming requests for information and responds to requests by opening mail, assisting walk-ins and telephone inquiries, and retrieving facsimile inquiries in a timely manner.


  • Conducts final review to assure medical records are complete, containing all documentation, reports, and forms that pertain to each admission/encounter.


  • Maintains charts, forms, and patient records in a neat and orderly manner at all times. Files charts and records accurately, neatly, and in a timely manner.


  • Assists with chart surveys, prioritizing and routing deficiencies to the appropriate individuals in prompt and efficient manner.


  • Routes documents to the Physician via DocuSign for provider signature and helps ensure compliance in this area.


  • Completes supply charge audit and/or enters supply charges into Athena in the interim of activation of the Jumpstock scanning process.


  • Provides excellent customer service by being attentive, respectful and professional at all times


  • Insures understanding of customer request and follows-through as promised; being proactive in identifying and addressing member concerns, or problems.


  • Maintains working knowledge of the current state laws regarding fee structure, and HIPAA regulations as well as facility policies and procedures regarding release of information as well as maintaining a thorough understanding of and utilizing Healthland and Athena EMR.


  • Always maintains patient and facility confidentiality and destroys confidential trash appropriately.


  • Works within scope of position and direction; accepts new assignments willingly to meet business needs.


REQUIRED QUALIFICATIONS


  • High school diploma or equivalency


  • Able to communicate effectively in English, both verbally and in writing


  • Good interpersonal skills dealing with patients, visitors, and employees


  • Clerical skills including filing and typing with high levels of accuracy


  • Basic to Intermediate Math Skills


  • Excellent organizational and time management skills


  • Basic to Intermediate computer skills in spreadsheets and word programs


PREFERRED QUALIFICATIONS


  • Experience working in a healthcare environment


  • Experience in a clerical / administrative position


  • CPR Certification preferred


  • Additional language skills - Spanish preferred


  • 10 key data entry


  • Typing speed 30-40 wpm
