The Consult Coordinator / LPN helps facilitate medical consultations and appointments for patients who require specialized care beyond the facility's capabilities. They coordinate with outside healthcare providers, schedule appointments, arrange transportation, and ensure that necessary medical records are prepared. Overall, the role of a Consult Coordinator or LPN in a correctional facility involves a combination of clinical care, coordination, and administrative tasks to provide essential healthcare services to inmates while adhering to the unique challenges of a correctional environment.


LPN / CONSULT COORDINATOR ESSENTIAL FUNCTIONS


  • The LPN / Consult Coordinator maintains accurate and up-to-date medical records for patients receiving specialized care. This involves documenting medical histories, test results, treatment plans, and communication with external healthcare providers.
  • LPN / Consult Coordinator acts as a liaison between the correctional facility's medical team and external healthcare providers. They communicate medical needs, coordinate information exchange, and ensure seamless collaboration for patients' medical care.
  • LPNs often conduct initial health assessments for newly arrived patients. They gather medical histories, assess current health status, and identify any immediate medical needs or concerns.
  • LPNs are responsible for administering medications to patients as prescribed by the facility's medical staff. They ensure proper dosages, document medication administration, and monitor inmates for any adverse reactions.
  • LPNs may provide basic health education to patients, covering topics such as hygiene, medication management, and preventive care. They help inmates understand their medical conditions and treatment plans.
  • In the event of a medical emergency, LPNs are trained to respond quickly and provide immediate care. They might assist with cardiopulmonary resuscitation (CPR), administer first aid, and help stabilize the situation until higher-level medical personnel arrive.
  • LPNs are often involved in wound care, including cleaning, dressing, and monitoring the healing progress of wounds or injuries.
  • Regular monitoring of patients' vital signs, such as blood pressure, pulse, and temperature, is a routine responsibility.
  • LPNs track any changes and report abnormal findings to the medical team.
  • LPNs might conduct routine health assessments, screenings, and health promotion activities for inmates. This can include providing vaccinations, conducting tuberculosis (TB) screenings, and promoting overall well-being.
  • Accurate documentation of all medical interactions, assessments, treatments, and interventions is essential.
  • LPNs maintain detailed records to ensure continuity of care and compliance with medical protocols.
  • The Consult Coordinator or LPN collaborates with other members of the medical team, including registered nurses, physicians, and mental health professionals, to provide comprehensive healthcare services to inmates.
  • LPNs may also be responsible for dispensing prescribed medications and ensuring that inmates receive the correct doses at the appropriate times.

LICENSED PRACTICAL NURSE MINIMUM REQUIREMENTS


  • Graduate from a Licensed Practical Nursing program.
  • Currently licensed as a Practical Nurse in the state of employment.
  • Possesses an active CPR certification.
  • Remains knowledgeable about specific state laws and regulations governing practice.
  • Satisfactory completion of initial and annual clinical competencies to demonstrate aptitude as assigned by role.
