About Hope Christian Health Center


Hope Christian Health Center is a non-profit, faith-based organization operating two clinics in North Las Vegas, a mobile outreach team, and the HCHC Recuperative Care Center (HCHC RCC) in Las Vegas.


The HCHC RCC is a 38 bed inpatient program that offers

primary care, case management, wound care, and other supportive services to clients who are homeless made possible through partnership with the City of Las Vegas.


Our mission is to glorify God as we demonstrate Christ's love to the Las Vegas community by providing excellent, affordable care to everyone regardless of their ability to pay.


JOB SUMMARY: The Bilingual Community Health Worker (CHW) supports patient health by providing education, outreach, and resource coordination. This role focuses on engaging high-risk patients, gathering relevant information, and collaborating with medical providers to develop personalized care plans. The CHW connects patients with clinic and community resources, promotes care plan compliance, and contributes to quality improvement efforts, ensuring patient-centered, comprehensive care.


DUTIES AND RESPONSIBILITIES:


Care Coordination and Case Management


  • Coordinate care and manage cases for patients, ensuring seamless navigation of health and social service systems.
  • Makes referrals and provides follow-up support to connect patients with needed resources and services.
  • Facilitates transportation services and addresses barriers that impede access to care.


Patient and Community Education


  • Educates patients and communities on navigating health and building health literacy and promoting cross-cultural communication.
  • Motivate patients in managing their health conditions and accessing necessary care.
  • Conducts client interviews to gather comprehensive data on medical history, socio-economic status, and program eligibility.


Advocacy and Resource Facilitation


  • Advocates for the needs and perspectives of individuals and communities, connecting them with appropriate resources and advocating for necessities.
  • Builds individual and community capacity through training sessions and collaborative initiatives with fellow CHWs and community partners.
  • Implements assessments to identify health needs and develop interventions.


Administrative Duties:


  • Protects HIPAA and maintains patient confidentiality.
  • Attends and actively participates in staff meetings
  • Maintains respectful, courteous, and appropriate communication and interpersonal skills with all staff members, volunteers, and patients
  • Supports patient care activities by managing time effectively, utilizing resources appropriately, and exemplifying excellence in attention to detail
  • Entering data into our EMR systems and other tracking systems
  • Handling calls from patients and community partners regarding patient needs
  • Performs all other duties as assigned

  • The Bilingual Community Health Worker (CHW) ensures compliance with all 19 Health Resources and Services Administration (HRSA) Health Center Program Requirements.
  • The Bilingual Community Health Worker (CHW) is also expected to actively participate in and support quality improvement activities and the clinical practice team efforts emphasizing the importance of the individual patient and putting the patient�s needs first.


QUALIFICATIONS:


  • High School Diploma or equivalent - required.
  • Fluency in English and Spanish - required
  • State of NV Community Health Worker Certification - preferred
  • Knowledge of Microsoft Office and experience with Excel spreadsheets - required


Education:


  • Highschool diploma or equivalent - required

TYPICAL PHYSICAL DEMANDS:


Occasional standing, walking, lifting, reaching, kneeling, bending, stooping, pushing, pulling, and lifting and/or moving up to 25 pounds. Frequent sitting. Manual dexterity. Ability to communicate by speaking and hearing in person or on the phone. Ability to travel to various locations. Ability and willingness to work outdoors during seasonal weather conditions.

Pay: $19.75 - $25.50 per hour
