Under the supervision of the Medical Director Ambulatory Services of Cook County (CCH), the Associate Medical Director is responsible for planning, implementing, monitoring, and assuring high quality medical services and clinical programs for the Ambulatory and Community Health Network (ACHN) centers of Cook County Health (CCH). This position will direct and/or assist in the coordination ACHN quality and safety initiatives, value-based programming, and patient centered care.


Typical Duties


+ Serves as the medical leader to help establish and guide the direction of a population health program for CCH.

+ Continuously works with other ACHN leaders to develop and refine the appropriate team-based model within the ambulatory sites with a strong focus on panel management, team organization, and high performance.

+ Leads and coordinates efforts related to the empanelment of CCH patients.

+ Assures the continuous preparation and oversight of regulatory readiness for providers working in the ambulatory sites.

+ Collaborates with Leadership across ACHN and CCH.

+ Enforces the new care paradigm, including productivity and revenue efforts.

+ Plans, participates, and represents ACHN in community-based activities.

+ Analyzes and supports opportunities to improve health outcomes and HEDIS metrics within the ACHN clinic sites.

+ Improves communication strategies with all the Medical Leads in ACHN to optimize communications with frontline providers.

+ Oversees the clinical workflows for primary care providers both clinically and via electronic record.

+ Participates in planning and implementing the ambulatory & health centers’ budget

+ collaborates with department and division chairs and other leaders in the improvement of patient care and throughput.to ensure continuous operational excellence.

+ Leads, conducts and/or assures the coordination and oversight of teaching and research activities at the health centers.

+ Responsible for the preparation of administrative reports.

+ Plans and implements new clinical programs; assesses and acquires the resources needed for the health centers’ programs.

+ Assure the supervision and evaluation of clinical staff.

+ Responsible in cooperation with the leadership team for planning and implementing the health center budget.

+ Leads or participates in the lead physicians’ meetings. Leads or participate in ACHN committees.

+ Oversees the performance of the primary care medical leads through coaching, site rounding, and evaluation.

+ Maintains an active clinical load of at least 60%.

+ Reports to the Medical Director, Ambulatory Services

+ Performs other related duties as assigned.


  • MINIMUM QUALIFICATIONS


  • Doctor of Medicine (MD) or Doctor Doctorate Medicine (DO) from an accredited medical college or university+ Licensed as a physician and surgeon in the State of Illinois prior to starting employment

+ Must be cleared for privileges by Medical Staff Services by start of employment

+ Board Certified in their specialty

OR Board Eligible in their specialty and must obtain Board Certification within two (2) years of starting employment


  • Five (5) years or experience of clinical and administrative responsibility in ambulatory carePreferred Qualifications


  • Experience in primary care practice

  • Knowledge of quality assurance principles and guidelines
  • Working knowledge of Microsoft Office including Word and Excel
  • Knowledge of and ability to comply with clinical documentation requirements, accreditation and regulatory agency requirements
  • Knowledge of ACGME requirements; ability to precept residents and teach health care professional students
  • Knowledge of IRB requirements
  • Knowledge of collaborative/supervisory agreement requirements for advanced practice providers
  • Excellent verbal and written communication skills necessary to communicate with all levels of staff and a patient population composed of diverse cultures and age groups
  • Strong interpersonal and empathy skills

  • Individual must be capable of travel to ACHN, CCH affiliates and community partner sites

This position is functioning within a healthcare environment. The incumbent is responsible for adherence to all hospital and department specific safety requirements. This includes but is not limited to the following policies and procedures: complying with Personal Protective Equipment requirements, hand washing and sanitizing practices, complying with department specific engineering and work practice controls and any other work area safety precautions as specified by hospital wide policy and departmental procedures.
