Petroleum Fuel & Terminal Company is seeking a
Terminal Operator for its Weirton, WV Terminal location. The Terminal Operator position is responsible for performing operational activities relating to the transfer of petroleum products between a variety of containers and maintaining tanks and facilities in accordance with regulatory compliance.EOE - Drug free workplace

· The two medical plan options are a Preferred Provider Organization (PPO) and a High Deductible Health Plan with Health Savings Account (HDHP-HSA)
· Preventive care covered at 100% in-network providers and diagnostic care is subject to deductibles and coinsurance for in and out of network providers for both plans
· Copayments apply to office, urgent care, and emergency room visits, prescription drugs for covered services with network providers, and deductible and coinsurance applies for other services under the PPO Plan
· Higher deductible and coinsurance apply to all covered services and prescriptions (in and out of network) and copayments apply to designated preventive medications under the HDHP-HSA Plan
· Health Savings Account available to enrollees in the HDHP-HSA Plan which allows participants to contribute in an HSA account on a pre-tax basis up to regulatory limits and the company matches a designated contribution annually
Two dental plan options are available through MetLife.
· The two dental plan options are a Base and a Buy-Up Plan
· The Base Plan covers preventive services for network providers at 100% and basic care subject to deductible and coinsurance with an annual maximum benefit
· The Buy-Up Plan covers preventive services for network providers at 100% and basic and major care subject to deductible and coinsurance up to an annual maximum benefit
· The Buy-Up Plan also covers a percentage of orthodontic care is covered for network providers up to a lifetime maximum benefit
Vision plan with two network options of VSP or Davis are available through MetLife.
· An annual eye exam is covered annually with a copayment for network providers and a designated allowance for out-of-network providers
· Lenses are covered annually subject to a copay for network providers and are not covered for out of network providers
· Frames are covered at with a designated allowance every twenty-four months for network providers and up to a designated dollar amount for out of network providers
· Contact lenses are covered every twelve months in lieu of glasses
Basic Life and Accidental Death and Dismemberment (Basic/AD&D) Insurance and additional life/AD&D insurance
· The company provides 1.5 times annual earnings up to a maximum amount in Basic/AD&D life insurance
· Additional life/AD&D insurance is available to purchase for employees and eligible dependents
Short-Term and Long-Term Disability Insurance
· The company provides Short-Term and Long-Term Disability insurance to eligible employees who will be off work for five days or longer due to a medical disability subject to waiting periods and maximum benefit limits
