Job Title: Social Worker (Opiod Settlement Program)


Job Summary:


The Social Worker (MSW) will conduct an assessment using our low-threshold screening and assessment instruments for co-occurring mental health disorders, substance use for people who use drugs.


Duties & Responsibilities:


  • Based upon assessment, the MSW will provide clients with individualized service planning staged upon client needs and client goals, offer clients choices, and provide access to support services (Recovery Support Services, HIV/HCV, medical, insurance enrollment, patient navigation, employment, education, housing) and linkage to a comprehensive range of evidence-based behavioral health services provided at BOOM!Health and Argus Community or another agency of client preference.
  • The MSW will regularly meet with participant�s in-person to review treatment plans and develop adjustments to treatment plans if needed.
  • The MSW will provide tobacco cessation counseling and treatment service referral.
  • Participates in program Performance Management meetings with supervisor.
  • Two years� experience providing services for populations affected by substance use disorder and co-occurring mental health disorders.
  • The MSW will have received training in the EBPs of Motivational Interviewing, Stages of Change, and Harm Reduction Theory, and in the provision of trauma-informed services.
  • Provide counseling, education and referrals for participants/clients identified as recently experiencing an overdose.
  • Provide service coordination for behavioral health needs in collaboration with agency programs and staff, external and community providers.
  • Provide individual behavioral health education sessions (maximum 3) to increase health literacy on the benefits of medications and engage in appropriate treatment.
  • Document all services provided for charting and reporting purposes.
  • Complete necessary documentation and maintain client records that document attendance, participation, progress, accomplishment and additional needs, referrals and goals.
  • Participate in overdose educational and naloxone training updates and overdose reversals.
  • Participate in quality improvement activities.
  • Attend required meetings, trainings, and supervision to stay updated on Overdose prevention and other relevant health matters.
  • NYS Mandated Reporter for Child Abuse and Neglect.
  • Bilingual preferred.
  • Reports to VP


Position Requirements:


  • Working knowledge of Harm Reduction models and practices
  • Experience with crisis intervention, including, risk assessments and incident management Experience with and knowledge of the provision of harm reduction services to high risk populations, including active drug users, the homeless and sex workers.
  • Excellent oral and written communication skills.
  • Ability to work successfully with a wide range of internal and external stakeholders
  • Demonstrated success in working in a fast-paced environment with multiple priorities Bilingual Spanish/English preferred


Education:


  • Master�s in Social Work required
  • Experience in harm reduction practices
