This key role involves ensuring the delivery of high-quality, resident-centered care, coordinating with interdisciplinary teams, and supporting the emotional and psychosocial needs of our residents and their families.
- Lead and supervise the social services team, providing guidance and support.
- Conduct assessments and develop care plans to address residents' emotional, psychological, and social needs.
- Collaborate with the medical team to ensure holistic care for residents.
- Advocate for residents' rights and facilitate communication between families, residents, and staff.
- Manage crisis interventions and provide counseling services as needed.
- Oversee compliance with federal and state regulations, ensuring social service standards are met.
- Provide support during discharge planning and transitions of care.
- Master�s Degree in Social Work (MSW) from an accredited program.
- Active Social Worker license in New Jersey (LCSW preferred).
- Experience in a healthcare setting
- Strong knowledge of state and federal regulations regarding social services in long-term care settings.
