The Service Coordinator (SC) is a service facilitator to residents. They may provide; limited case management; information and referral; plan educational and health programs, and coordinate volunteer opportunities. The SC links with outside service agencies and negotiates affordable services as needed. The SC educates residents on available services and develops a network of contacts with service providers and agencies for resident referrals. The SC also plans social activities for the residents to participate in on site.
The Service Coordinator works in conjunction with the Community manager and other management staff of the facility to empower residents to age in place and remain as independent and self-reliant as possible.
1. Welcomes new residents to explain to them the services provided.
2. Assists and educates residents and families of the services which may be necessary to maintain self reliant life style
3. Promotes wellness activities for all residents.
4. Educates residents on in informal support networks among themselves and with family members.
5. Acts as a liaison between community agencies, service providers and residents.
6. Works as a team member with Community Manager and other management staff in serving residents.
7. Encourages residents to be proactive in meeting their social, psychological and physical needs.
8. Facilitates meeting of needs when necessary but avoids the creation of unhealthy dependence.
9. Uses the least restrictive intervention necessary to alleviate a problem situation.
10. May assist residents or coordinate training for residents in understanding lease and tenancy obligations.
11. Does not perform any duties or function that is duly assigned to management or is associated with management responsibilities.
1. Serve daily DSA lunch Monday - Friday, and complete related paperwork if applicable.
2. Collaborate with the Activities Committee to organize social events and outings.
3. Educates residents on services availability, application procedures, and resident’s rights.
4. Sets up volunteer support programs with service organizations in the community.
5. Advocates and may negotiate on behalf of residents for adequate timely and cost effective provisions of services.
6. Meets with services providers as needed and appropriate with resident’s approval.
7. Assembles a directory of community services and makes it available to resident’s families and management.
8. Assists management in identifying residents who need assistance.
9. Facilitates and organize all resident meetings, community organizing and social events.
Organizes and oversees all volunteers.
