Delta Health is seeking a compassionate, experienced, and proactive
RN Patient Navigator to join our Emergency Department team. This role is ideal for a Registered Nurse who thrives on collaboration and care coordination and is passionate about providing patient- and family-centered care. As an RN Patient Navigator, you will guide patients through the continuum of care in the ED, serving as a critical link between patients, families, and the interdisciplinary care team.+
BLS certification (current upon hire)+
ACLS, TNCC, and PALS/ENPC certifications (required within 6 months of hire)+ Case Management experience
+ Meditech experience
+ Strong analytical and problem-solving ability
+ Effective multitasking and team collaboration skills
