Assignment Duration: 13 Weeks with Possible extensions(Tuesday Start Dates)
Shifts: Days | 40 hours/week guaranteed
Weekend Requirement: Rotating weekends as needed (2-3 per month)
On-Call Requirement: 6 shifts/month (including weekends)
Pay Rate:
- $62/hour on w2 (For Locals)
- $68/hr on w2 (For Traveler's)
We are seeking two experienced and compassionate
Registered Nurses (RNs) for a 13-week Home Health assignment in KÄne‘ohe, Hawaii. The ideal candidate will demonstrate strong clinical knowledge, autonomy, and the ability to coordinate care in a home setting. You will be responsible for managing complex patients and supporting positive outcomes through effective case management and collaboration with interdisciplinary teams.- Provide direct patient care in a home health setting following individualized care plans.
- Coordinate care with physicians, social workers, and the broader care team to ensure quality and timeliness of services.
- Monitor and manage patient progress and report changes or concerns proactively.
- Serve as a liaison among patients, families, providers, and payers to optimize care outcomes.
- Utilize OASIS assessments and Homecare Home Base software to document visits and track progress.
- Participate in care planning, patient education, and chronic care management.
- Support transitional care including discharges and transfers to higher levels of care when needed.
- Manage resource utilization and ensure services are in alignment with covered benefits.
- Lead and consult on the management of high-utilization patients, including those with chronic pain, substance use, or homelessness.
- Participate in on-call rotation and work rotating weekends as required.
- Maintain compliance with state, federal, and agency standards, including infection control and vaccination policies.
Minimum of 6 months of recent Home Health experience - Required*
Previous travel nursing experience - Required*
Active Hawaii RN license - Required*
Valid driver's license and auto insurance - Required*
Proficiency in Homecare Home Base and OASIS documentation - Required*
COVID-19 and Flu Vaccinations - Required (No Exceptions)*
Bachelor’s degree in Nursing - Preferred*
Prior Case Management experience - Preferred*
