Job Overview

We are seeking a dedicated and compassionate Registered Nurse Case Manager to join our healthcare team. The ideal candidate will be responsible for coordinating patient care, ensuring that patients receive the necessary services and support throughout their healthcare journey. Duties


  • Assess patient needs and develop comprehensive care plans tailored to individual requirements.
  • Coordinate and manage patient care across various settings including inpatient, outpatient, and emergency services.
  • Facilitate discharge planning to ensure a smooth transition from hospital to home or other care facilities.
  • Collaborate with healthcare providers, patients, and families to provide education on treatment plans and available resources.
  • Monitor patient progress and adjust care plans as necessary based on ongoing assessments.
  • Advocate for patients� needs within the healthcare system to ensure they receive appropriate services.
  • Maintain accurate documentation of patient interactions, care plans, and outcomes in compliance with regulatory standards.


Requirements


  • Current Registered Nurse (RN) license in good standing.
  • Experience in emergency medicine, critical care, or behavioral health is preferred.
  • Strong background in patient care with knowledge of geriatrics, PICU, primary care, and inpatient settings.
  • Proficiency in discharge planning processes and coordination of care across multiple disciplines.
  • Excellent communication skills with the ability to work effectively in a team-oriented environment.
  • Ability to manage multiple priorities while maintaining attention to detail.

Pay: $24.47 - $60.00 per hour
