The RN Care Manager plays a vital role in supporting patients and providers within the primary care setting by coordinating comprehensive care management services for patients with chronic illnesses, high-risk conditions, or complex health needs. This role emphasizes patient-centered care, promoting health outcomes, reducing hospitalizations, and enhancing the quality of care through education, care planning, and follow-up.


Essential Duties and Responsibilities


  • Collaborate with primary care providers and multidisciplinary teams to develop, implement, and monitor individualized care plans.
  • Conduct comprehensive assessments of patients� physical, mental, and social health needs.
  • Provide education and self-management support for patients with chronic diseases such as diabetes, hypertension, asthma, COPD, and heart failure.
  • Identify and address barriers to care, including social determinants of health, and make appropriate referrals to community resources or specialists.
  • Facilitate transitions of care, including hospital discharge planning and follow-up coordination.
  • Monitor and follow up on test results, medication adherence, medication management, and treatment compliance.
  • Serve as a liaison between the patient, provider, family members, and healthcare system.
  • Document all interactions and care plans accurately in the electronic health record (EHR).
  • Participate in quality improvement initiatives, care team huddles, and population health management strategies.
  • Ensure compliance with regulatory standards, HIPAA, and professional nursing practice guidelines.


Minimum Qualifications


Registered Nurse with a Maryland State license required.


CPR certification required


Certified Case Manager (CCM) Preferred


Registered Nurse with 3 years of experience to include 1 or more years of proven case management and population health coordination experience. Candidate must have strong analytical and critical thinking skills.


Candidate must be computer literate and fluent in Microsoft Outlook, Word and Excel. Experience with eClinicalWorks, data collection and reporting preferred. Must have effective verbal and written communication skills. The candidate must have excellent interpersonal and customer service skills as the position serves geriatric patients.
