Rising Star to join our Support Services department. This is a meaningful opportunity for professionals who are passionate about helping others live healthier, more independent lives. As a Rising Star, you’ll work directly with individuals to assess their needs, develop personalized care plans, and connect them with essential services and supports. You’ll be part of a mission-driven team that values your expertise, supports your growth, and empowers you to make a real difference in your community.- Work a minimum of 20 hours per week.

Rising Stars will work directly with our community’s most vulnerable to assist them obtaining community based in-home health services. In addition, Rising Stars gain leadership experience by participating in, and occasionally leading, cross-training efforts to ensure adequate coverage for all support services tasks and departmental projects to improve the operations of the business. Further, Rising Stars will be responsible for providing coordination of the Support Services Pool by utilizing in-depth knowledge of company functions to effectively schedule assessments and provide support services to internal and external clients.
