Rehabilitation Counselor - Enhanced In-Home Support Program

• Develops Treatment Plans and identifies the child�s barriers to community integration and develops strategies to offset them in consultation with the child and their family.
  
• Collaborates with the parents/guardian of the child/adolescent regarding approval for all therapeutic activities/interventions.
  
• Attend Individuals Education Plan meetings (IEP meetings) as required to serve as an advocate for the child�s needs in the school system.
  
• Assist in planning and coordination of all rehabilitation activities including self-care skills, medication monitoring, health promotion, safety, communicable disease prevention, crisis intervention and other areas as needed with individual consumers.
  
• Provides linkages and access to the necessary resources, entitlements, and supports to ensure community integration.
  

• Bachelor�s Degree in Social Work, Psychology, Rehabilitation Counseling, or a related discipline is preferred
  
• Driver�s license, proof of auto insurance, and access to a vehicle for work purposes
  
• Knowledge of basic computer applications (Google Suite, Microsoft Office) and ability to learn software systems and/or websites (Paycom, EHR, Relias, etc.)
  
• Additional experience must consist of at least one (1) year in direct service

Pay: $40,000.00 - $42,000.00 per year
