Full-Time. 9am - 5:30pm. Candidate will coordinate with the interdisciplinary team, patient/family on complex aspects of the patients' care and serves as the focal communication point for patients and staff.
Graduate from an accredited school of nursing (BSN preferred); Current licensure in the State of PA; Minimum of two years of clinical experience with strong clinical skills; Solid computer skills; Demonstrated leadership skills; Knowledge of Core Measure and Meaningful Use requirements as they pertain to patient care preferred; Strong understanding of Meditech EMR functions preferred; Supervisory experience preferred; Certification in Case Management (or equivalent) or within one year.
