We have an excellent opportunity for a Full Time Registered Nurse (RN) for a very flexible position in home health. This position offers a lot of flexibility, as the Nurse (RN) schedules directly with their own patients, allowing you flexibility and the ability to schedule around occasional personal appointments as necessary. The Registered Nurse (RN) MUST have at least one year of experience with adult/geriatric patients such as in the hospital and/or home care setting. The pay and benefits for this Registered Nurse (RN) position are very generous. This Registered Nurse (RN) position is in the Home Health setting doing homecare visits with adult and geriatric patients. • Current State of Connecticut Registered Nurse License
  
• At least one (1) year of clinical experience
  
• Home Health or Home Care experience is Preferred
  
• Current driver's license with Reliable transportation
  
• Strong clinical assessment, organizational, and communication skills
  
• Ability to work independently and as part of a team.
  
• Provides skilled hands-on home health care to patients in the comfort of their own homes
  

• Develop the patient's plan of home care for the safe and effective delivery of home health services
  
• Work closely with an interdisciplinary team to achieve optimal patient outcomes
  
• Provide education to empower patients to manage their chronic disease processes
  

Job Type: Full-time

Pay: $78,000.00 - $95,000.00 per year
