Key Responsibilities Patient-Centered Care & Clinical Excellence


  • Conduct comprehensive initial assessments to determine individualized home care needs.
  • Develop, implement, and continuously evolve patient-centered care plans.
  • Perform ongoing nursing assessments and adjust care plans in real time based on changing conditions.
  • Administer treatments, medications, and interventions as prescribed-bringing hospital-quality care to the home.
  • Empower patients and families through health education and hands-on guidance.
  • Support proactive discharge planning to ensure seamless care transitions.
  • Serve as Case Manager when designated, coordinating holistic care and ensuring continuity.


Communication & Collaboration


  • Maintain clear, timely, and accurate clinical documentation.
  • Collaborate closely with physicians, care coordinators, and interdisciplinary teams.
  • Serve as a vital liaison between the patient, family, and healthcare providers.
  • Participate in case conferences and advocate for optimal patient outcomes Additional Duties
  • Participate in on-call rotation for 24/7 care continuity.
  • Coordinate necessary equipment, services, and resources for effective home treatment.
  • Supervise and evaluate home health aides every 14 days, ensuring top-tier care delivery.


Qualifications & Must-Haves


  • Graduate of an accredited nursing program (Associate or BSN preferred).
  • Active RN license in the state, current CPR certification, and valid driver�s license.
  • 1-2 years of acute care experience; home health care experience preferred.
  • Strong communication, critical thinking, and clinical documentation skills.
  • Capable of lifting, positioning, and transferring patients-hands-on and ready for action.
