The Registered Nurse Diabetes Program Manager will adhere to and complete all of the requirements of the SDPI or Special Diabetes Program for Indians contract issued to the Helena Indian Alliance, as the overall program manager. The RN shall focus care management of diabetes patients by ensuring the clinical quality measures are addressed as well as best practice interventions for diabetic care. The RN shall conduct educational opportunities for patients and the community regarding diabetes care. The role is integrated with the medical clinic.


DUTIES, ROLES AND RESPONSIBILITIES


  • Responsible for overall management, grant development and implementation process of the SDPI Program
  • Provide diabetic patient assistance by developing treatment plans and make appropriate referrals to outside treatment facilities and other medical professionals. Modify treatment plans to comply with changes in patient status.
  • Plan, design and implement diabetes prevention and treatment education programs for individuals and groups.
  • Plan and implement educational services to the community, maintain and develop outreach activities as assigned.
  • Completes and maintains accurate and detailed records for all monthly, quarterly, semi-annual, and annual reports per reporting requirements.
  • Coordinates with medical Clinicians to provide clinical care, chart scrubbing, and coordinating overall care for diabetic patients.
  • Coordinates clinical operation as it pertains to the SDPI Program
  • Establishing and implementing policy and practices that ensure the effective and efficient delivery of services to clients and customers that are consistent with current nursing and medical practice.


· Uses relevant nursing research to improve clinical practice.


· Practices according to clinic policies, procedures, and protocols, and practice guidelines to maintain a safe environment for patients, visitors, and staff.


  • Participates in the quality improvement strategies for diabetic clinical quality measures and participates in monthly calls with the Billings Area Diabetes consultant.
  • Serve as key advisor to the Executive Director regarding overall Leo Pocha SDPI policy of the Helena Indian Alliance.
  • Performs sterilization of all diabetic foot tools and demonstrate proper care of all equipment
  • Assist with other clinic patients as needed.
  • Performs retinopathy exams; Performs diabetic foot care; Performs phlebotomy and lab testing


· Promote and support HIA through other activities in fundraising, professional representation, public appearances


· Attend monthly staff meetings and education and training opportunities


· Additional duties as assigned by supervisor and/or Executive Director


KNOWLEDGE, SKILLS, ABILIITES, AND USE OF EQUIPMENT


· Establish working relationships with staff, patients/families and public.


· Knowledge of general clinical practices.


· Excellent communication and inter-personal skills


· Excellent ability to operate a computer, software programs, electronic medical record systems, and clinical applications


· Knowledge and understanding of the Native Indian culture and traditions


QUALIFICATIONS, EDUCATION, TRAINING, AND EXPERIENCE (REQUIRED OR DESIRED)


· Registered Nurse with unencumbered license in the State of Montana


· At least two years of experience in a clinical or medical setting performing direct patient care.


· Preferred to have knowledge or experience in treating diabetic patients


· Current certification in BLS. May complete within 3 months of employment


· Valid Montana Driver’s License with a driving record that will allow coverage on the center’s insurance policy.


PHYSICAL DEMANDS


Work is performed in a medical and behavioral health office and clinic setting; sitting, with intermittent standing and walks; reaches for and uses writing instruments and keyboard; reads reports and other written materials; extensive use of telephone and oral communication with the public and coworkers; stoops, bends, kneels, reaches for, picks up, and pushes or pulls; ability to lift up to 25 pounds.


WORKING ENVIRONMENT


All employees of Helena Indian Alliance may have contact with patients, families, visitors that may be physically debilitated, have mental or psychological conditions, or may be under the influence of a substance. Employees work areas include the entire facility and its property. Employees will have interaction with the entire staff throughout the facility. The position may require off-site work at times and may work after hours or weekends for program needs.
