Job Title PCP/QMAP Manager


  • Full-time.
  • 6am-2pm, and every other weekend.
  • Starting wage: $18.50-22.00/hr. depending on experience.

About the Role:


As a PCP/QMAP Manager at Columbine Commons, you will:


  • Provide direct care to assigned residents, assist with medication administration, and ensure residents' belongings (e.g., hearing aids, glasses) are accounted for by the end of the shift.
  • Communicate resident status changes to the LPN/RN and ensure all required documentation is completed.
  • Coordinate breaks for staff, respond to calls and pages, and welcome new residents to the facility.
  • Attend required meetings and trainings, and provide orientation and training to new staff members.
What We're Looking For:


  • 1+ year of experience as a care provider required.
  • Completion of 16-hr. Personal Care Provider training offered through CHS required.
  • Qualified Medication Administration Professional required.
  • CPR certification required.
  • Must be at least 17 years of age.
  • Interest in geriatric population preferred.

Our Total Rewards package includes a salary range of between $18.50 - $22/hr. DOE
