Summary


The Paramedicine Coordinator at Decatur County Hospital plays a vital role in ensuring patients' health care needs are met by providing in-home visits for chronic illnesses, post-surgical follow-ups, medication management, home safety reviews, and more. This position is integral to supporting our mission of delivering exceptional patient care and enhancing the overall efficiency of our hospital.


Brief Description of Job:


The Community Paramedicine Program Coordinator will work in the Med/Surg, Outpatient Clinic, Surgery/Infusion, Emergency, and other departments as needed to ensure that patients have a quality experience at Decatur County Hospital, including during home visits for qualified program participants who live with chronic illness(es).


Provides direction for the hospital’s utilization review. Provides assistance for the hospital’s social services and care coordination. Works collaboratively with physicians, staff and other health care professionals within his/her network to provide comprehensive care management and care coordination across the health care continuum, especially for patients living with chronic illness(es). Is an integral member of the health care team who works to ensure safety, best practice and high quality standards of care are maintained. Also works to ensure members are able to navigate through the health care continuum by improving the coordination of care and member/family experience. Coordinates a wide range of self-management support services as well as a wide range of community-based and health care support services for members. Works with physicians, staff and other resources to address specific quality improvement/performance improvement initiatives.


Specific Duties


Community Paramedicine Program Coordinator


  • Work closely with our staff and providers in order to identify patients who may be eligible for the Community Paramedicine Program.
  • Suggest ideas and solutions that will enhance the care experience.
  • Collaborate in the design, implementation, and monitoring of the Community Paramedicine program initiatives to create and sustain a culture focused on high quality care, free from preventable harm delivered in a patient-centered, team-based way.
  • Ensure consistently patient-centered care by partnering with caregivers to exceed the expectations of patients and families.
  • Continually review opportunities to improve the patient experience by monitoring real-time patient feedback and collaborate with individual departments to review feedback and assist with prioritizing patient experience improvement objectives.
  • Report and address patient feedback.
  • Implement patient-focused programs in a home care setting.
  • Generate and deliver reports on patient progress, setbacks, and overall status for Community Paramedicine patients.
  • Work to accomplish assigned goals on a regular basis as determined by department needs.
  • Respond to patient queries and concerns in a professional manner.
  • Document and discuss patient grievances and resolutions with Administration, formulate and execute action plans to resolve.
  • Follow hospital policies and provide exceptional patient services.


Educational/Experience Requirements:


Is an, LPN, RN, or Paramedic professional who -


  • Is licensed or registered, if applicable, by the State in which practicing; and
  • Is eligible for certification as a clinical professional by a recognized accrediting body on or after October 1, 1990; or
  • Has 2 years of experience in a Paramedicine role within the last 5 years, 1 of which was full-time in a health care setting; or
  • Has completed a training course approved by the State. Has BLS certification, and Child & Dependent Adult Abuse certification.

Job Type: Full-time


Pay: $25.00 - $35.00 per hour
