Job Description: PAB/Community Learning Service Worker
*CARE Hawaii Inc. is a Behavioral Health Care Organization seeking a compassionate and dependable Crisis Direct Care Staff worker to help support a residential behavioral program.
Primary location: Island Wide
· Document and maintain Data collection for client’s progress in regards to behaviors, incidents, health plans, home and community objectives.
· Transport client from home into the community setting safely
· Provide supportive assistance with client’s in home and community daily living needs
· Contribute as a team member in treatment planning for clients
· Demonstrate effective communication with client,family members, and Service Supervisor.
· Work in a 1:1 setting in the home setting and in the community to execute responsibilities delegated by Service Supervisor.
· Meet once a month in the community setting with Service Supervisor for monthly supervision.
· High School Diploma or equivalent
· 6 months experience working with Developmenta lDisabilities
· Full time and Part time hours available
· Day, Mid , and Night shifts available
