Requirements:
- No more than 7 years old
- CDL-A Driver Requirements:
- Veltri, Inc. Application
- Current Driver�s License/Current DOT Physical/Medical Certification Card
- Complete a drug-screening test
- Background verifications with a minimum of 3 years� driving experience.
- Permission to verify previous employers and drug & alcohol testing verifications.
- Clean MVR
- Owner Operator Requirements:
+ Copy of most recent truck inspection
+ Copy of 2290 Highway use tax form
+ Copy of Truck Title - front and back or copy of the bill of sale/purchase agreement
+ Copy of IFTA Registration (If not elected)
+ Certificates of Insurance - Listing Veltri, Inc. as Certificate holder (If not elected)
o Non-Trucking Liability
o Physical damage
o Occupational accident
+ Copy of Registration (if you do not elect our Tag/Plate Program)
+ W-9 (Provided)
+ Direct Deposit Form (Provided)
+ Executed Lease Agreement (Provided)
