Overview


We are seeking dedicated and skilled Nurse Practitioners or Physician Assistants to join our WOUND CARE teams covering the TULSA METRO, Greater Tulsa Metro Area and surrounding communities in Northeast Oklahoma. The ideal candidates must have WOUND CARE experience and will provide high-quality patient care in private residences for at-home patients, but may also include some long term care and skilled nursing facilities. This role requires strong clinical skills, the ability to work collaboratively within healthcare teams and an unwavering commitment to patient-centered care. No nights and weekends. Active Oklahoma RN/APRN or PA license required.


Key Responsibilities


  • Conduct thorough physical examinations and comprehensive health assessments for patients presenting with wounds
  • Develop and implement individualized treatment plans based on the latest clinical guidelines and best practices
  • Utilize diagnostic tools such as imaging studies and laboratory tests to aid in diagnosis
  • Perform WOUND CARE procedures, including dressing changes, debridement and wound closure techniques
  • Prescribe appropriate medications, including antibiotics and pain management therapies
  • Collaborate with physicians and other healthcare professionals to ensure coordination of patient care
  • Monitor wound healing progress and adjust treatment plans as necessary
  • Educate patients and caregivers about health conditions, treatment options and self-care strategies to promote healing
  • Active involvement in utilization management in order to optimize patient outcomes and ensure effective use of resources
  • Manage discharge planning for patients transitioning from hospital to home or other care settings
  • Ensure compliance with healthcare regulations, safety standards and infection control practices


Qualifications


  • Active APRN or PA license in the state of Oklahoma
  • Proficiency and/or certification in the provision of WOUND CARE and WOUND CARE MANAGEMENT
  • Excellent clinical assessment skills with the ability to perform necessary procedures
  • Strong communication skills for effective patient education and collaboration with healthcare teams.
  • Ability to manage multiple priorities in a fast-paced environment while maintaining attention to detail
  • Proficiency with multiple Google platform applications and EHR/EMR software (Intellicure)
