What You'll Do


  • Conduct in-home health risk assessments for members in your geographic area
  • Evaluate past medical history, review systems, reconcile medications, and perform vital signs and physical exams
  • Use evidence-based practice screening, including point-of-care testing as needed
  • Identify diagnoses for care management by Porter Care Guides
  • Actively manage medical treatment based on assessment findings
  • Formulate lists of current and past medical conditions using clinical knowledge and judgment


What You'll Need


  • Valid unencumbered South Carolina NP license
  • 3 years of patient care experience required (experience in primary care/adult/geriatric, and EMR skills preferred)
  • A strong sense of empathy, compassion, and friendliness
  • Exceptional communication skills, both written and oral, with the ability to assist others (patients and coworkers alike) with sincerity, respect, and compassion
