- Participate in care coordination and regular interdisciplinary team meetings
- Demonstrate adaptability when handling urgent or changing patient needs
- Maintain compliance with all clinical protocols, regulations, and quality standards
- Focus on patient-centered care, especially for complex or high-needs patients
- Provide a positive, supportive attitude toward all care team staff to promote a healthy work environment
- Licensed Nurse Practitioner (NP) in the state of Idaho
- At least 1 year NP experience; home health or primary care preferred
- Current CPR certification
- Valid driver�s license with reliable, insured vehicle
- Strong assessment, communication, and organizational skills
- Able to work independently in a home setting
