Key Responsibilities:* Provide care through clinical judgment, diagnostics, and treatment planning

• Manage acute and chronic conditions in the SNF setting
  
• Maintain clear, complete documentation to support continuity and quality of care
  
• Participate in team meetings, peer reviews, and continuous learning initiatives
  

Qualifications:* Graduate of an accredited NP or PA program; eligible for Idaho licensure

• DEA and prescriptive authority (or ability to obtain)
  
• Strong interest in internal medicine, geriatrics, and transitional care
  
• Excellent communication and collaboration skills
