Position Summary:


The Medical Coordinator is responsible for overseeing and coordinating all medical and health-related services for individuals receiving services under the New Mexico DD Waiver. This role ensures timely access to medical care, adherence to physician recommendations, compliance with DD Waiver and state regulatory standards, and effective communication among interdisciplinary team (IDT) members, including guardians, case managers, and healthcare providers.


Essential Duties and Responsibilities:


  • Coordinate all routine and specialized medical appointments for DD Waiver participants.
  • Ensure implementation and documentation of physician orders, including labs, diagnostics, medication changes, and treatments.
  • Track and maintain current medical records, immunizations, screenings, and annual exams in accordance with DD Waiver requirements.
  • Communicate health status updates, changes, and concerns to appropriate IDT members, including families and guardians, while maintaining HIPAA compliance.
  • Follow up on hospitalizations, emergency room visits, and discharges to ensure continuity of care.
  • Collaborate with agency nurses, direct support staff and individuals served to ensure medical and health-related supports are provided effectively.
  • Monitor and ensure proper medication administration and storage procedures in coordination with nursing and program staff.
  • Assist with preparing for and participating in internal and external audits and quality assurance reviews.
  • Maintain accurate and up-to-date documentation in electronic health systems (e.g., Therap, iTherapyDocs, or other agency-used platforms).
  • Participate in IDT meetings as needed, advocating for individuals� health and wellness.
  • Provide training and support to staff on medical documentation, incident reporting, and emergency medical procedures.


Qualifications:


  • High school diploma or GED required;
  • Minimum of 2 years� experience in healthcare coordination, case management, or a related field, preferably within the DD Waiver system.
  • Knowledge of DD Waiver standards, ISP process, and medical documentation requirements in New Mexico.
  • Strong organizational and time management skills with the ability to manage multiple priorities.
  • Proficiency in electronic medical records and Microsoft Office Suite.
  • Strong interpersonal and communication skills; ability to work collaboratively with diverse teams.


Additional Requirements:


  • Must pass criminal background check and other screenings required by the New Mexico Department of Health (DOH) and DDSD.
  • Valid New Mexico driver�s license, reliable transportation, and insurance (for travel to appointments or visits).
  • Completion of all DDSD-required trainings and certifications within required timeframes (e.g., HIPAA, ANE, CPR/First Aid, Rights and Advocacy).
  • Availability for on-call or after-hours follow-up as needed.

Work Environment:


  • This position may require travel between agency sites, medical appointments, and individuals� homes.
  • Office work with frequent interactions via phone, email, and in-person with medical professionals and IDT members.
  • May require occasional lifting of medical supplies or assistance during appointments.


Job Type: Full-time


Pay: $15.00 - $16.00 per hour


Expected hours: 40 per week
