The
MDS Nurse, Registered Nurse, RN is responsible for coordinating the MDS functions within the facility. Job responsibilities may include but are not limited to:- Coordinate and implement the Resident Assessment Instrument (RAI) process in relation to all federal, state, and facility guidelines.
- Schedule and complete the Minimum Data Set (MDS) for residents.
- Be an active part of the interdisciplinary team.
- Develop, review, and revise the comprehensive person-centered plan of care.
- Be a resident advocate where the resident is the center of all we do.
- Ensure excellent communication among residents, families, and staff.
- Create positive resident and staff outcomes.
- Respect others by treating them the way we want to be treated.
- Demonstrate kindness, compassion, honesty, and integrity at all times.
- Help create a culture of superior customer service.
- Valid Registered Nurse (RN)
- Successful criminal background check
- Subject to pre-employment drug testing
- Long-term care facility experience preferred
- MDS experience required
