Position Summary: (12,000 Dollar Sign on Bonus!!)


The MDS Coordinator is responsible for the coordination and completion of the Resident Assessment Instrument (RAI) process, ensuring accurate and timely completion of Minimum Data Set (MDS) assessments and care plans in accordance with federal and state regulations. This position plays a key role in optimizing quality of care, regulatory compliance, and reimbursement.


Essential Duties and Job Responsibilities:


  • Coordinate and manage the completion of MDS assessments, Care Area Assessments (CAAs), and care plans for all residents in accordance with CMS guidelines and state regulations.
  • Accurately capture residents� clinical status, services provided, and needs through thorough documentation and chart reviews.
  • Collaborate with interdisciplinary team members (nursing, therapy, dietary, social services, etc.) to ensure timely and accurate assessments and care planning.
  • Monitor changes in residents� conditions and ensure appropriate MDS updates and modifications are completed.
  • Submit MDS data electronically to CMS and ensure successful transmission with error-free validation reports.
  • Audit resident records for accuracy and completeness; ensure appropriate documentation supports MDS coding.
  • Participate in Quality Assurance and Performance Improvement (QAPI) programs and assist with quality measures monitoring.
  • Educate and train nursing and clinical staff on accurate documentation and MDS-related procedures.
  • Maintain knowledge of regulatory changes and ensure compliance with all applicable rules and policies.
  • Assist with Medicare/Medicaid reimbursement processes by ensuring correct MDS coding related to case mix or PDPM models.
  • Attend care conferences and contribute to the development of individualized care plans.
  • Assist in Floor Nurse duties/be on call if necessary


Qualifications:


  • Current RN license in the state of Wyoming in good standing.
  • Minimum 1-2 years of experience in MDS coordination in a skilled nursing facility.
  • In-depth knowledge of MDS 3.0, RAI process, PDPM, and applicable federal/state regulations.
  • Strong organizational and communication skills.
  • Proficiency in EMR systems and MDS software (e.g., PointClickCare).
  • Ability to work independently, meet deadlines, and manage multiple priorities.
  • Must be able to work 40 hours a week in house
