WORK SCHEDULE: This position works 3 days (24 hours) per week with a schedule of Wednesday 8am-5pm, Thursday 7am-4pm & Friday 7am-4pm.

ABOUT THE POSITION: The Triage Nurse assists Interdisciplinary Teams with the daily management of patient care by processing phone and fax information from patients, caregivers, physicians, Hospice staff and other health care providers. The Triage Nurse assists Clinical Supervisors, Director of Social Services and the Medical Director with patients’ plan of care, case management and other projects as assigned. The Triage Nurse may also make emergent visits as necessary, advising and informing the Primary Nurse regarding client assessment and recommend care measures. In the emergent capacity, the Nurse will provide prompt response to physical and emotional needs of patients and their families in the form of phone call and/or home visits during regular day shift hours; they screen a variety of questions, problems, and crisis situations that may arise.

MINIMUM QUALIFICATIONS: LPN required; current Idaho State LPN in good standing; two (2) years nursing experience in acute care setting (Hospice, home health preferred); current CPR; valid drivers license and evidence of insurance coverage as required by HONI policy required.
