Job Summary

We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will provide high-quality patient care in various settings, including hospitals, nursing homes, and assisted living facilities. The LPN will work under the supervision of registered nurses and physicians, assisting in the assessment and treatment of patients while ensuring compliance with healthcare regulations and standards.


Responsibilities


  • Provide direct patient care, including administering medications, performing wound care, and monitoring vital signs.
  • Assist in the development and implementation of individualized care plans based on patient assessments.
  • Conduct patient assessments and document medical histories accurately in electronic health records (EHR) systems.
  • Collaborate with healthcare team members to coordinate patient care and ensure optimal outcomes.
  • Educate patients and their families on health management, medication administration, and post-discharge instructions.
  • Maintain a safe and clean environment for patients by adhering to infection control protocols.
  • Participate in triage and emergency response situations as needed.
  • Support patients with developmental disabilities, Alzheimer�s care, memory care, and other specialized needs.
  • Perform IV infusions, catheterization, phlebotomy, and other nursing procedures as required.


Experience

Candidates should possess experience in one or more of the following areas:


  • Critical care experience or ICU experience at a Level I or Level II trauma center.
  • Long-term care experience in nursing homes or assisted living facilities.
  • Hospital experience in acute care settings including emergency medicine or hospital medicine.
  • Familiarity with medical terminology, ICD coding (ICD-9/ICD-10), and medical documentation practices.
  • Experience working with geriatric populations or providing hospice care is preferred.
  • Proficiency in EMR systems such as Epic, Cerner, eClinicalWorks, or Athenahealth is advantageous. The successful candidate will demonstrate strong clinical skills, effective communication abilities, and a commitment to providing exceptional patient-centered care across diverse healthcare environments.

Pay: From $42.00 per hour
