We are currently seeking a professional that possesses strong leadership and interpersonal skills to join our exceptional and welcoming centralized admissions team. Clinical Liaison is responsible for assessing patients for admission to post-acute and long-term care, as well as outreach to our referring partners.


In this position you will:


  • Increase census through cultivating relationships with our hospital partners to facilitate safe and effective transfers, develop relationships and grow admission and census levels.
  • You will attend hospital meetings and will network to grow brand awareness.
  • Promote quality and cost-effective outcomes and a seamless process for post-acute admissions by communicating with nurses, hospital case managers and all available resources.
  • Following up on outstanding items needed to close medical records
  • Corresponding with various departments to ensure efficient transitions of care


Other responsibilities may include: Answering inquiries regarding admission in a timely, enthusiastic and positive manner, Verifying insurance coverage, completing internal documentation and preparing medical records for admission.


Skills & Certifications:


  • Active NJ LPN License
  • Must be willing to rotate weekends
  • Experience with admission screening or case management or strong knowledge of medical conditions and terminology
  • Strong assessment skills through review of medical records
  • Excellent communication and interpersonal skills
  • Experience in care transitions in a hospital or skilled nursing; Post-Acute experience preferred.
  • Must be a self-starter and have a passion for success!
