Position Summary:


The Lead PEP and PrEP Navigator serves as a frontline advocate and support for clients seeking HIV prevention services, including Post-Exposure Prophylaxis (PEP) and Pre-Exposure Prophylaxis (PrEP). This role combines

direct client navigation with community-based outreach. The Navigator works closely with clinical staff, community partners, and public health teams to increase awareness, access, and retention in PEP/PrEP services.


Key Responsibilities:


Client Navigation


  • Provide education and counseling on PEP and PrEP.
  • Triage and assist clients in determining eligibility for PEP or PrEP services.
  • Conduct testing, including phlebotomy
  • Coordinate appointments, follow-up testing, and lab work with the clinical team.
  • Support clients with insurance navigation, financial assistance applications (e.g., 340B, manufacturer copay cards), and adherence strategies.
  • Maintain ongoing contact with clients to support medication adherence and retention in care.


Outreach and Engagement


  • Conduct community-based outreach in high-need settings (e.g., LGBTQ+ centers, events, bars/clubs, shelters).
  • Build and maintain relationships with community organizations to increase PEP/PrEP referrals and awareness.
  • Deliver presentations and educational materials
  • Participate in mobile testing or health events as part of the clinic�s prevention outreach efforts.


Program Support


  • Document all client encounters and follow-up in the electronic health record (EHR) and program databases.
  • Participate in regular case reviews, staff meetings, and training sessions.
  • Track outreach efforts and client outcomes for program evaluation and grant reporting.
  • Help identify and address barriers to care for Spanish-speaking clients and other underserved populations.


Qualifications:


Required:


  • Strong understanding of and sensitivity to issues affecting LGBTQ+ and HIV-impacted communities.
  • Experience in outreach, health education, case management, or navigation in a healthcare or community setting.
  • Excellent communication, organizational, and interpersonal skills.
  • Ability to work flexible hours, including evenings or weekends for outreach events.


Preferred:


  • Knowledge of HIV prevention strategies, including PEP, PrEP, and harm reduction.
  • Familiarity with sexual health, STI testing, and/or public health navigation services.
  • Experience working with uninsured or underinsured populations.
  • Proficiency with EHR systems or case management software.

  • Compensation: The salary for this position is $42,000.

  • Paid Time Off: Full-time employees are eligible for up to 15 days of PTO (prorated based on start date), 2 Floating Holidays, plus 8 Holidays annually and the clinic closes the week between Christmas Eve and New Year�s Day.
