About the Role:


Sierra Hospice, a division of Sierra Health Care, Inc., is seeking a full-time

Hospice Social Worker and Bereavement Coordinator to serve patients and families in Truth or Consequences and the surrounding communities. As part of a small, trusted team in a nonprofit setting, this dual-role position offers meaningful work, flexible scheduling, and the opportunity to support individuals and families during some of life’s most vulnerable moments.


Key Responsibilities:


  • Provide psychosocial assessments and counseling to patients and their families throughout the hospice journey
  • Coordinate care planning with the interdisciplinary team (IDG) to support emotional, social, and spiritual needs
  • Develop and manage the bereavement program, including follow-up services for families for at least 13 months after a loss
  • Facilitate grief support groups and design educational materials related to loss and coping
  • Supervise and coordinate bereavement volunteers and ensure services align with regulatory standards
  • Maintain accurate documentation and contribute to quality and compliance efforts
  • Participate in interdisciplinary team meetings and community engagement efforts
  • Serve as a liaison to community resources, agencies, and support services


Requirements:

What We’re Looking For:


  • Master�s degree in Social Work (MSW) from an accredited institution
  • Experience in hospice, palliative care, or medical social work preferred
  • Specialized training or demonstrated experience in grief counseling
  • Ability to work independently with sound clinical judgment
  • Strong communication skills and a compassionate, professional demeanor
  • Understanding of cultural sensitivity and the needs of diverse populations
  • Current driver�s license and reliable transportation
