Schedule: Full-time | Fri/Sat/Sun 8 AM- 8 PM
Location: Dayton, Ohio (surrounding area)
- Licensed RN in state of practice
- BLS Certified
- A minimum of 1 year of Clinical experience
- Valid Drivers License and Auto insurance Coverage
- Reliable transportation: Willing to travel
- On-call/ triage experience preferred
- Provide on-call services after hours, and complete skilled visits if needed.
- Coordinate hospice care by working with other team members as well as with the patient's family members to ensure needs are met.
- Triage calls, coordinates services, and provides frequent updates on plan of care and any changes in condition.
- Assist reinforcement of patient and family education regarding end-of-life care.
- Participate in IDG and case conferences when needed
- Assume responsibility for personal and vocational growth and development through active participation in nursing organizations and participating in in-service programs and other on-the-job learning.
- Keep the RNCM informed of changes noted during the process.
- Participate in self-development by practicing further education, workshops, and conferences, active in professional and related organizational meetings, and individual research reading.
- Respond to on-call or medical emergencies (nights and weekends)
