Position Summary


We are seeking an experienced and innovative

Hospice Liaison to join our team. This role is ideal for someone who deeply understands hospice philosophy, communicates with transparency, and can think creatively to meet the unique needs of patients and referral sources. The ideal candidate will never over-promise and will always deliver with compassion and professionalism.


Key Responsibilities


  • Build and maintain strong relationships with hospitals, skilled nursing facilities, Assisted Living facility, physician�s office, and other referral sources.
  • Educate healthcare professionals and the community about hospice services and the philosophy of end-of-life care.
  • Assess potential patients for hospice eligibility and coordinate with the clinical team for timely admissions.
  • Serve as a trusted resource for patients and families, offering clear, honest, and compassionate guidance.
  • Collaborate with the interdisciplinary team to ensure smooth transitions and continuity of care.
  • Develop creative outreach strategies to connect with underserved populations and referral partners.
  • Represent Advantage Hospice with integrity, always aligning with our mission and values.


Qualifications


  • Minimum of 1-2 years of experience in hospice, palliative care, or related healthcare field.
  • Deep understanding of hospice philosophy and end-of-life care.
  • Strong communication and relationship-building skills.
  • Ability to think outside the box and adapt to changing needs.
  • Clinical background (RN, LVN, MSW, etc.) preferred but not required.
  • Self-motivated, compassionate, and ethical.
