Home Health Aid/CNA

The Home Health Aide (HHA) provides personal care and assistance with activities of daily living (ADLs) under the supervision of a Registered Nurse (RN) or Licensed Practical Nurse (LPN). This role plays a vital part in helping patients remain safe, comfortable, and independent in their own homes.

• Assist patients with personal care including bathing, dressing, grooming, toileting, and oral hygiene.
  
• Help patients with mobility, transfers, and walking, including the use of assistive devices.
  
• Provide support with meal preparation, feeding, and light housekeeping related to patient care.
  
• Observe, report, and document patient conditions, changes, and concerns to the supervising nurse.
  
• Measure and record vital signs as instructed (temperature, pulse, respiration, blood pressure).
  
• Follow the patient�s individualized care plan and maintain compliance with Medicare and agency standards.
  
• Promote a safe home environment and assist in fall prevention.
  
• Maintain patient confidentiality in accordance with HIPAA and agency policies.
  

• High School Diploma or equivalent (preferred).
  
• Completion of a state-approved Home Health Aide or Certified Nursing Assistant (CNA) program.
  
• Active HHA or CNA certification in the state of IL.
  
• Current CPR certification (preferred).
  
• Previous experience in home health or patient care setting is a plus.
  
• Strong communication skills and compassion for patients.
  
• Ability to travel to patient homes within the service area.
  

• Ability to lift up to 50 lbs, bend, stoop, and assist patients with mobility.

Pay: $21.16 - $35.00 per hour
