Position Function Summary

Under the direction of the Chief Operating Officer, the HCH Program Director is responsible for planning, organizing, and directing the day-to-day activities of La Familia Health’s (LFH) Health Care for the Homeless program (HCH) and medical clinic; and responding to the health and case management needs of patients by developing, implementing, and managing programs that address the problems and needs associated with the condition of homelessness.

Essential Responsibilities

• Ensures appropriate health care, case management, and referral services, as well as the general well-being of LFH/HCH homeless patients by developing, implementing, and continued evaluation of the HCH program with special attention to the problems associated with homelessness, by ensuring that:
  
• Service delivery (hours, location, language capacity, etc.) and mix of services are responsive to the needs of homeless individuals in Santa Fe County.
  
• Program includes homeless individuals in the development and oversight of the program in order to foster patient-centered approach to treatment.
  
• Individuals experiencing homelessness and those at risk are aware that services are available and that they obtain all assistance need to access these services the above activities are effectively integrated so that they support homeless individuals in achieving housing stability.
  
• Ensures that LFH/HCH is a community leader on homeless issues and health care by working with homeless services providers, collaborative partners, community services, health care providers, and other community entities to facilitate clinical affiliations and by overseeing outreach to the community, training of volunteers, and coordination of referrals from LFH/HCH providers to community supports and services;
  
• Develops and expands current and new innovative programs that align with the increasing need of medical and behavioral health services in the Santa Fe Metro and County.
  
• Oversee the day-to-day operation of all service sites and assist in the facilitation of the Street Medicine program. Evaluate the effectiveness of all service delivery sites, as well as identify opportunities for improvement. Collaborates and seeks regular feedback from the HCH team as well as the patients to identify gaps and recommendations to improve service delivery.
  
• Working with the CFO and CDO, correctly manages funding deliverables. Seeks and applies for funding opportunities to support initiatives aimed at closing gaps in care of the unsheltered and at-risk populations.
  
• Expand the role of HCH in medical service provision within Santa Fe County. This includes additional service sites and the expansion of staff to align with the increasing need for services.
  
• Aligns with community partners to develop new ways to address the needs of the unsheltered
  
• Maintains professional and technical knowledge by participating in continuing education or seminars related to job responsibilities with approval by supervisor
  
• Ensures involvement in Value Based Payment Reform Program and is familiar with performance measures associated with each Medicaid contract. Works with Quality Manager to pull reports and provide data to provide to Medicaid providers to ensure quality measures are being met at HCH
  
• Contributes toward operational success of LFH by developing, monitoring, and complying with the budget for the HCH, participating in quality improvement initiatives, and continually evaluating performance indicators delineated in the business and clinical plans for LFH;
  
• Directs and supervises reporting personnel in such a manner as to promote a safe working environment, safe operation of LFH, productive efficiency of employees, and managing the Department in keeping with state and federal laws and LFH�s policies, guidelines, and EEO/affirmative action policies;
  
• As a member of the LFH management team, participates in strategic and operational planning; translates the goals, strategy and programs of LFH into specific and meaningful work assignments for the department, delegating assignments and responsibility to direct report staff in order to accomplish program goals and objectives;
  
• Leads personnel in such a manner as to promote a productive, efficient, and safe working environment; establishes an effective flow of communication and information to ensure that problems can be responsibly solved and that support is provided to the staff as needed. In coordination with the human resources department and supervisor, is responsible for the selection, assignment, training, development, review and performance management of assigned direct reporting personnel;
  
• Ensures implementation of an effective performance management process for all direct reporting staff that clearly defines performance expectations and measures, including goal and objective setting, performance feedback, accountability measures, and corrective action as appropriate, and in compliance with sound management practices and applicable laws and regulations to ensure high levels of organizational, work unit, and individual performance.
  
• Coordinates with health education program to provide services, identify target health issues and geographic areas to be served and prioritize health education goals and services for identified communities as well as individual patients;
  
• Coordinates with Chief Clinical Officer to ensure safety and functionality of clinical systems by overseeing maintenance and repair of all medical and nursing equipment at HCH
  

Requirements:

Education, Experience and Skills/Training

• Master�s degree plus 2 years of supervisory experience or bachelor�s degree plus, five (5) years supervisor experience from an accredited institution required. Background and education in Social Work or Public Health preferred. In lieu of a degree, must possess sufficient prior work experience in progressively more responsible positions during which the skills and abilities to perform all the essential functions have been amply demonstrated at a consistently competent level.
  
• CPR certification required.
