Gilead Community Services is seeking an attentive and recovery-oriented Social Rehabilitation Counselor to join our multi-disciplinary program staff.
The Social Rehabilitation Counselor for Gilead’s Middletown area adult social rehabilitation program works 40 hours: M, T, W, Th, F 7:30a-3:30p providing case management, advocacy, and outreach in a program serving adults with major mental illness. The individual in this role carries out all phases of social support service including intake, engagement, assessment, treatment planning, case management, referral, and discharge. This role also requires the timely completion of required documentation including assessments and other treatment planning.
This is a union position and pays $18.89-20.69 per hour dependent on level of education.
