This includes evaluating residents, identifying areas of concern and coordinating services with the interdisciplinary team as well as contacting community resources as needed. The Director of Social Services will have a strong managerial background as he/she will oversee case management services for residents and review resident status.
Required Education and Experience
- Bachelor�s Degree in social work (SW) or social welfare from an accredited academic institution
- At least 2-years experience in health care setting
- Excellent written and verbal communication skills
- Computer proficiency with Microsoft products
Preferred Education and Experience
- Current state license as a Social Worker
- Experience in outpatient rehabilitation preferred
- Experience in a fast-paced environment
- Post-acute care or health care experience preferred
