The CNA/CMT is a key person responsible for delivering direct care to clients in an in-home setting.
Essential Functions:
Under direct supervision of the RN Case Manager
· Follows POC (plan of Care) as completed and maintained by RN
· Assist Clients with ADL/IDL in, in-home setting. This includes the following: Feeding, Bathing, Toileting, Dressing/Grooming, Nail Care and Cleaning of occupied areas
· Complete documentation on care given for each visit. This includes the following: Taking Vital Signs, Temperature, Record intake of medications/food and Record Bowel movements.
· Report all changes of the patient to RN and Care Coordinator.
· Properly and safely lift, move, transfer and position clients in bed, wheelchairs and while using other equipment.
· Accompany residents on appointments and act as an advocate for resident with providers.
· Interact with clients, families and visitors in a warm, caring and courteous manner.
· Travel within 30 minutes of home.
Education: HS Diploma or GED
Licensing: Active and unencumbered CNA/CMT licensure in MD/ Compact States
Maintain CPR/First Aid
Physical Demands: Stooping, turning, bending, squatting, kneeling and the ability to lift up to 50 pounds; constant/repetitive walking and standing; requires normal, correctable vision and hearing, and the ability to accurately discern color as necessary to perform job functions

Clients have gloves and masks at home. Office provides PPE.
