Case Manager

We are seeking an independent contractor with experience in case management for a new, consumer-directed model of care program in Cherokee County, Oklahoma, for Veterans to live in their homes and hire their own caregivers. The case manager/options counselor will arrange appointments with the Veteran, provide education on the program, prepare service plans and budgets to meet personal care needs, conduct monthly phone monitoring, make quarterly in-home visits, ensure adherence to program timelines, and monitor the spending plan. Flexible days/hours that meet the Veteran's schedule. This position requires strong communication skills, computer literacy, math skills, attention to detail, and reliable transportation. The rate is $20.00 per billable 15-minute unit. Ideal position for an experienced case manager needing weekend or after-hours work.

• Arrange initial home visit with the Veteran and their caregivers, address inquiries, and provide guidance on the Veteran Directed Care (VDC) program.
  
• Provide options counseling services in accordance with the service standards as set forth in the Veteran Directed Care Field Guidebook.
  
• Meet all timelines established by Contractor and the Veteran Directed Care Program relative to Options Counseling.
  
• Ensure that each Veteran�s personal care services and activities of daily living needs are met through service plan development.
  
• Complete all VDC documentation on the appropriate form provided by Ability Resources and other data as proof of delivery of service required by the Veterans Administration.
  
• Submit documentation within 10 days of the activity date.
  

• Current or previous experience as an ADvantage Program Case Manager.
  
• Strong math skills.
  
• Exceptional communication skills, both verbal and written, to interact effectively with team members.
  
• Proficiency in using Word and Excel software programs.
