Case Managers are part of a team of professionals including therapists, nursing, vocational and support workers. We have a position available in our Bradford office.
We have a shared company vehicle, but use of your own vehicle will also be necessary.
- Documenting clinical services in an electronic medical record.
- Gathering & completing paperwork to open charts, document progress and discharge when appropriate.
- Providing outreach & support to clients in the community and their homes.
Job Type: Full-time
Pay: $22.00 - $24.00 per hour
Expected hours: 40 per week
