Enrollment Case Manager Team and client is targeting the following counties: 2 Allegheny, 1 Cambria, 2 Lancaster, 1 Mifflin/Perry/Synder, 1 Mckean/Elk, 2 Montgomery, 1 Northampton, 6 Philadelphia, 1 Schuylkill, 1 Westmoreland

(Numbers shows open position in each county)

The hiring event will take place on Aug 18-21, and the new hire class start date for this event would be Monday, 09/08/25.

Compensation: $20.50/Hr. monolingual

Education: A minimum of one year experience in public or private social work; or a minimum of two years of case work experience including one year of experience performing assessments of individuals functional ability to determine the need for institutional or community-based services; and a Bachelor’s degree, with or supplemented by a minimum of 12 semester hours credit in sociology, social welfare, psychology, gerontology, or other related social science; Or a Bachelor’s degree with a social welfare major; Or any equivalent combination of experience or training with successful completion of a minimum of 12 semester hours credit in sociology, social welfare, psychology, gerontology, or other related social science.

a. One-year experience in a local AAA office may be substituted for one-year assessment experience.

Experience: Excellent organizational, written and verbal communication skills; ability to interact courteously and effectively with a variety of audiences, including the disability community; ability to handle pressure and perform multiple tasks; ability to work independently; experience entering data into automated information systems; knowledge of local health care networks and providers; ability to maintain cultural sensitivity

The candidates must be willing to travel throughout their assigned county and complete in-home visits.

• There may be instances where ECM�s are expected to, and scheduled to, travel outside of this radius as dictated by business needs.
  
• ECM�s work primarily in applicants' places of residence.
  
• They will conduct intake visits for waiver applicants and act as a facilitator for the eligibility determination.
  
• Must have their own car. Public transport is not allowed.
  

o Travels throughout the assigned county and neighboring counties as dictated by business needs, working primarily in applicants' places of residence

o Conducts intake visits for HCBS applicants and acts as a facilitator for the eligibility determination process, providing assistance from the time individuals are identified as potential participants in an HCBS program to the time they are enrolled as participants or are terminated from the intake process

o Presents and discusses the concept of HCBS programs, and advises applicants of their rights and responsibilities

o Assists with application submission, including data entry of the Compass Application

o Assists applicants with selecting an MCO, Life Provider or Support Coordinators that best meets their needs, ensuring consumer control is maintained throughout the application process

o Assists with developing relationships with community-based organizations, advocates and stakeholders to gather feedback on improving the application process and removing barriers to enrolling in HCBS waiver programs

o Fosters and maintains relationships with parties serving our mutual consumers, including physicians, County Assistance Offices, providers, nursing homes and rehabilitation facilities

o Facilitates the efficiency of the application process, ensuring any stalled cases receive extra attention and support

o Attends regularly scheduled meetings

o Reports on schedule availability at regular intervals

o Occasionally lifts, carries, or otherwise moves items weighing up to 25 pounds.

Job Types: Full-time, Contract

Pay: $19.00 - $20.00 per hour

Expected hours: 40 per week
