Care Manager
Full-time position (37.50 hours weekly) that provides assessment for our in-home services; investigates reports of older adults who may be neglected, abused and/or financially exploited; conducts comprehensive assessments; develops and implements crisis intervention strategies to remove/reduce the risk and help ensure safety; performs follow-up monitoring.
Candidates must be detail oriented and possess excellent communication and reasoning skills, strong documentation skills, and good computer skills. Must be able to follow time frames for documentation.
Educational Requirements -- Bachelor’s degree with a social welfare major; or a Bachelor’s degree which includes or is supplemented by 12 credits of college level courses in sociology, social welfare, psychology, gerontology, or other related social sciences and 1 year experience in public or private human services setting; or at the sole discretion of the agency any equivalent combination of experience and training including successful completion of 12 credits in sociology, social welfare, psychology, gerontology, or other related social science.
Work hours are primarily Monday - Friday 8:30am - 4:30pm, with occasional after hours as part of a paid on-call rotation schedule. On-call once every fifth week, and additional pay of $150 for on-call week. All Care Managers have a private office and ability to use agency vehicles for travel daily.
Pre-employment and random drug tests required.
