Care Coordinator, Rapid Response Services


EPIC is currently seeking a Care Coordinator for our Rapid Response Team for adults in St. Johns County, FL. The Team will provide high quality de-escalation, assessment, counseling, care coordination, and peer recovery services to adults in need of immediate mental health and/or substance use community services.


This position will be Monday through Friday responding to adults in need of immediate mental health services.


As a Care Coordinator, Rapid Response Team you will assist individuals with services and supports that they need to transition successfully from higher levels of care to effective community-based care. This may include connecting clients to behavioral health, peer recovery supports, housing, education, vocation, and the justice systems. The Care Coordinator is an important member of the clinical team working collaboratively with clinicians and peer recovery specialists.


This is a full-time position and includes the following responsibilities:


· Assess an individual’s level of care determination to plan and access other ancillary health and human community resources that address their social determinants of health (housing, employment, healthcare, benefits, etc.).


· Develop an individualized care coordination plan with each client that emphasizes self-management, recovery, and wellness.


· Provide effective transitions and warm hand-offs to community and interpersonal relationships that promote the individual’s achievement of treatment goals and objectives.


· Monitor the client’s behavior, schedule, and activities for successful treatment outcomes.


· Assist individuals in arranging medical appointments, support groups, and other resources to aid in the participant’s recovery.


· Evaluate and respond to any urgent crisis of clients and/or their families and provides crisis intervention to connect clients to appropriate services.


· Meet with individuals/families in the office, community settings, or their homes as indicated by supervisor or client need. Contact can also include telecommunication.


· Provide information and education to family members and significant others of clients to link with needed resources to help improve the quality of life for the client.


· Complete documentation of all client services in the company’s electronic health record system.


MINIMUM QUALIFICATIONS


Bachelor’s Degree in psychology, sociology, social work, counseling education, criminal justice or other relevant discipline.


  • Minimum of one (1) year experience providing assessment, behavioral management and case management linkages to community resources.
  • Current valid driver�s license in the State of Florida


· Successful completion of Level 2 background screen and drug screen.
