Care Coordinator

Job Purpose & Position Overview:The purpose of this position is to help identify patients that are in need of assistance. This position will assist with case management of patients to assist with referrals to the appropriate programs and will maintain follow-up with those patients. This position will assist clients to determine if they are eligible for alternate resources such as Medicaid, Medicare, private insurance and others. This includes determining if the patient qualifies for alternate resources. This position will assist the patient with completion of the application. This position will also follow up with the insurer to ensure the coverage is completed.
Specific Job Duties/Responsibilities:* Demonstrates the Winnebago Tribe’s core mission, vision, and values.

• Accept referrals, calls and walk-in clients from the community and its agency partners.
  
• Gather information from incoming referral source to assess the needs of the family to better identify and refer to appropriate programs, services, or supports including but not limited to; job services, substance abuse treatment, tribal enrollment, housing, transportation, education attainment, etc.
  
• Follow up with providers/family to assure all intake information is received to assess their need, goals and eligibility for other services and supports.
  
• Coordinate scheduling with client and provider to remove barriers and ensure follow through for services.
  
• Conduct outreach activities to inform agencies and families in the area of the initiative and available services resources.
  
• Attend trainings, presentations, and meetings regarding protective factors and family centered practices.
  
• Knowledgeable and sensitive to issues about trauma and its effects and supports trauma informed changes in service delivery.
  
• Carry a client caseload.
  
• Maintain accurate caseload information to report data back to funder(s).
  
• Identify patients who are eligible for alternate resources.
  
• Assisting the patient with completion of the application.
  
• Following up with the insurer to assure the coverage.
  
• Maintain statics of enrollment, approvals/denials of application, verification and overall assistance of patients with benefits.
  
• Effectively interview patients to determine eligibility and make appropriate referral for benefits.
  
• Make home visits to gather pertinent information/documents from clients/patients to complete application process.
  
• Performs other duties as assigned for the purpose of the funding sources.

Qualifications/Skills:

• Preference will be given to those with an AA or Higher Degree. Minimum high school diploma or verifiable GED certificate with 2 years of experience.
  
• Must have a valid driver�s license and must be able to meet driver�s eligibility guidelines set forth by the Winnebago Tribe of Nebraska�s insurance carrier to become an insured driver.
  
• Must have advanced computer skills.
  
• Proficiency in Microsoft Office Word, Excel, and Publisher.
  
• Must have excellent communication skills and organizational skills.
  
• Excellent verbal and written communication skills, including strong organizational, detail and interpersonal skills.
  
• Must possess or willing to acquire Medical Terminology knowledge.
  
• Ability to maintain confidentiality.
  
• Ability to make decisions in accordance with established policies and procedures.
  
• Ability to work independently, to work under supervision, and to work in a team-based and goal-oriented environment.
  
• Possess a valid State Driver�s License and must meet driver�s eligibility guidelines.
  
• Must be able to pass pre-employment drug screen.

Physical Requirements:

• Normal to light office work which includes the ability to lift up to 10 lbs.
  
• Regularly required to walk, stand, and sit for extended periods of time and use hands to operate computer keyboard, phone, and various office equipment.
